Unsigned Ballot Statement

If you did not sign the return envelope as required by California Election Code §3011, our office mails the Unsigned Ballot Statement form.

  1. Please read the instructions.
  2. Fill out this form.
  3. Return the form by 5 p.m. on November 17, 2021.

If you are no longer able to sign the envelope on your own, you can make a “mark” and have it witnessed by a person over the age of 18 for this election. The witness’ name and signature should appear next to the mark and be noted as witness. For future elections, you can re-register to vote with your mark on file as your legal signature. If you are not able or willing to make a mark, you may re-register using a signature stamp.

Signature Verification Statement

If your signature on the envelope does not match the signature in your voter registration record, you will be mailed the Signature Verification Statement.
