Unsigned Ballot Statement
If you did not sign the return envelope as required by California Election Code §3011, you will be mailed the Unsigned Ballot Statement form.
Please read the instructions before you fill out Unsigned Ballot Statement form form. Return this form by 5 p.m. on December 1st, 2020.

Candidate Filing Documents
Candidates running for office for the November 3, 2020 Presidential General Election should complete the Candidate Registration Form and email it to Michelle Yue, Candidate Service Lead at MYue@smcacre.org. Upon receipt of the completed Candidate Registration Form, our office will call the candidate to set up a filing appointment to complete the candidate fling process. Every San Mateo County voter receives a Vote by Mail ballot. It is important to keep your information up to date so we can ensure your ballot reaches you.
If you have changed where you receive your mail, but have not changed your residence, use this form to give us your new mailing address.
If you want to remove a mailing address you provided to us previously, complete the form and enter “none” under Mailing Address.
